Steps to Reproduce

1. We fought phase 1 and 2 completely normal
2. We depleted phase 3's armor and put it into its invincible state
3. The altar did the map wide shockwave but didn't spawn the fragments
4. We looked all over and couldn't find them so we gave up and let the endless horde kill us

Explanation

Molaktula gatekeeper didn't spawn shattered orb fragments, softlocking the boss and forcing us to lose the run
