Steps to Reproduce
- I joined a match as a guardian
- I got downed by a teammate with a cluster grenade
- They were unable to save me before it was time for everyone to get on the elevator
- I think my mid-match death might have triggered it
Explanation
I'm not sure if I can recreate it, but when I joined a game that had already started, after the mission was over it said we failed, but we all beat the boss and were standing with plenty of health.
