Game ended with "Lost facility" after loading into elevator on new floor with 3 dwarves downed at end of last floor. Host reported they dont know what caused end of game.

A game counted the mission as failed, even though there was 1 player remaining alive

I'm not sure if I can recreate it, but when I joined a game that had already started, after the mission was over it said we failed, but we all beat the boss and were standing with plenty of health.
