Mission Failed After Loading Level 2 Despite Successful Revival

Steps to Reproduce

1. Team goes down on level 1, at least one was captured
2. I revive after a teammate goes down reviving me
3. I rush to the elevator, we get all the between-floor picks
4. We load in to level 2, and immediately get a 'Facility Lost' screen

Explanation

I was on the elevator to level 2, rest of my team down but I was able to get on the elevator after using Retcon's auto-revive, but the moment level 2 loaded in we got a 'mission failed' screen and were kicked out.
