After loading into the match, all four perk slots were empty. My friend also couldn't see any perks on my character, so it wasn't just a visual issue. I played the entire match without any perks.

Steps to Reproduce
- Launch Dead by Daylight.
- Queue for a match in the CHAOS SHUFFLE event.
- Load into the match.
- Check your perk slots at the start of the trial.
- Notice that no perks are assigned.
Explanation
I played a match in the CHAOS SHUFFLE event, and after loading into the game I noticed I had no perks at all. All four perk slots were empty. I asked my friend, who was in the same match, if he could see any perks on my character, but he couldn't either.
I ended up playing the whole match without any perks, even though the event is supposed to give you four random perks at the start of the match.
