Perk stopped activating after first use. I recharged my item to 99% and once depleted was unable to recharge again

Explanation
While using the perk "Built To Last", the perk occasionally stops working after the first occurrence of recharging your item. (Rather than being able to recharge the item to 66%, it fails to activate when entering a locker, even if the item is fully depleted)
I have noticed this occurring on random occasions with Built To Last using different items (flashlights and toolboxes). I was using the masquerade toolbox on the last occurrence. My other perks at the time included Deja Vu, Fast Track, and Adrenaline (so nothing that should cross-interact)
