The Shape's "slaughtering strike" will commonly teleport the killer into facing another direction. Happens often around the proximity of vaults and pallets with a survivor nearby.

When playing Myers, the slaughtering strike randomly switches the player's direction

I was playing Myers and I charged my Slaughtering Strike and went for a lunge, only to teleport way off target and whiff. Happens pretty much every game. Sucks.
