Steps to Reproduce

Drive in front of David's auto in the right lane
Driving the most expensive challenger with a passenger

Explanation

While driving in game, me and my passenger were randomly removed from my vehicle and at the bottom left of my screen a note popped up, claiming I received a 0 dollar payout after a insurance claim. I haven't crashed from the point in which I spawned the car and was following the road laws. I was operating the most expensive challenger which was insured.
