This video demonstrates the generator was fully completed and I successfully escaped the match, so the stated achievement requirements were satisfied. There also appear to be reports from other players experiencing similar issues with this achievement and other map-specific achievements that require completing a particular generator and escaping. Because reaching Fallen Refuge can require a significant number of matches, reproducing the achievement attempt repeatedly is difficult. I have consumed all of my Ichorous Loam offerings and have had to have teammates use theirs.

Steps to Reproduce
- Launch Dead by Daylight through Steam.
- Enter a public Survivor match on Fallen Refuge.
- Complete the generator associated with the Fallen Refuge achievement.
- Successfully escape the trial.
- Check Steam achievements after the match.
Explanation
The Steam achievement "Don't Open, Dead Inside" does not unlock after completing the stated requirements.
I completed the generator located at the Fallen Refuge during a public match and successfully escaped the trial (through hatch). Despite completing both requirements, the achievement did not unlock on Steam. This is the second time I have completed the achievement on my account since the map launched and the achievement has not unlocked.
To ensure this achievement wasn't just staying locked due to a lag time, I played multiple survivor and killer matches afterward. Shut down the game and Steam to relaunch everything and the achievement still has not unlocked.
